Express using SMART format to ensure outcomes are measurable.

o Medium-term column (outcomes) includes the specific intermediate changes that are desired for health program participants. Express using SMART format to ensure outcomes are measurable. o Long-term column (outcomes/impact) includes the specific ultimate changes that are desired for health program participants. Long-term outcomes are often the drastic changes in health status (of participants or community at large) that occur about 5+ years after the start of a health program.
